Lake Michigan College (LMC or the College) may require a student to submit documentation as part of the enrollment process, to clear a discrepancy on the account, or as part of other processes related to student recordkeeping.

Documentation submitted for any purpose is subject to review and approval as noted: 

Document Delivery Method Approver

Test Scores (CLEP, SAT.  ACT, AP, etc)

Must be sent to LMC from originator Registrar
Transcripts - college Must be sent to LMC from originator Registrar
Transcripts -high school Must be sent to LMC from originator Registrar
Social Security card (copy) May be provided by student Registrar
Driver's license, State ID, passport, or other photo ID (copy) May be provided by student Registrar
Documents related to I-20 application, other than transcripts May be provided by student International Student Spec.
Proof of address May be provided by student Registrar
Birth certificate (copy) May be provided by student Registrar
Tax information May be provided by student Director of Financial Aid
Proof of residency (tuition)  May be provided by student Registrar
Documents related to appeals May be provided by student Registrar or Dir. of Financial Aid
Documents related to military benefits of policies May be provided by student Veteran Student Spec.
Signatures (faculty, staff or student) May be electronic or physical; may be provided by student or required to be provided by signor Registrar or Dir. of Financial Aid

The above is not to be considered a comprehensive list.  Any documentation that is requested may be reviewed to determine whether it is acceptable for the process, authentic, and considered official.
